I certify that all statements made by me on this application for employment are true and correct to the best of my knowledge and belief, and are a full and complete disclosure. If it should be discovered that the information I am providing is inaccurate, misleading, or incomplete in any respect, I will be disqualified from employment at J.B. Steel or, if hired, I will be terminated. All information is subject to verification. I authorize and consent to my current and prior employers, educational institutions and persons or organizations named in this application (or accompanying resume or supplements) to release any information to J.B. Steel or its representative that may be required to make an employment decision. This authorization will serve as a release, and for this purpose, a photocopy or facsimile shall be considered an original and valid. I understand and authorize a complete background check, in the form of an investigative consumer report, or prior employment and personal history, including but not limited to, my character, general reputation, personal characteristics, and mode of living. I understand that I can obtain a complete and accurate disclosure of the nature and scope of the investigation requested by submitting a written request to J.B. Steel.

IF HIRED: I understand that if I am hired by J.B. Steel, this application will be transferred to and made a part of my personnel file. I understand that employment is contingent upon the following, if required by J.B. Steel: successful completion of a physical examination and pre employment drug screen, administered by a J.B. Steel selected doctor; a transcript of high school and college grades; and copies of other pertinent documents including, but not limited to, W-2 statements. I understand and agree that all products or results of any work which I may do in the course of or in furtherance of my employment with J.B. Steel, whether of a patentable nature or not, shall be the sole property of J.B. Steel, and any copyright which may be applicable or patent which may be applied for relating to such work-product or result shall be unconditionally assigned to J.B. Steel or its designee. I further understand that any knowledge whatsoever of a confidential and/or proprietary nature which I may acquire as the result of or in connection with my employment shall remain the sole and exclusive property of J.B. Steel both during and after termination of my employment, and I hereby promise not to disclose or discuss such information, at any time, except to authorized company personnel or other persons authorized in writing by the company. I understand that any handbooks, manuals, policies or procedures maintained by J.B. Steel are not contractual in nature and may be amended or abolished at the sole discretion of J.B. Steel at any time. I understand that no representative of the company, other than the President, has any authority to enter into any agreement for employment for any specified period of time or to promise any other personnel action, either prior to commencement of employment or after I have become an employee, or to promise any benefits or terms and conditions of employment, or make any agreement contrary to the foregoing. I understand that if employed by J.B. Steel, such employment is not for any definite period but may be terminated by either party at any time, and any offer of employment, or my acceptance of an employment offer, if such is to occur, may be withdrawn, with or without cause, and with or without prior notice, at any time, at the option of the company or myself.
